Presented is the prenatal sonographic detection of a case of symelic bipodia sirenomelia associated with bilateral renal agenesis. This is the rarest form of sirenomelia (mermaid syndrome), and the diagnosis is important as the condition is lethal. Termination of the pregnancy is generally recommended for these cases, regardless of gestational age.
