Abstract
In this article we discuss the implications of a few critical drawbacks associated with the present state of Long-term and Post-Acute Care processes. This is discussed in some detail using a hypothetical example. Using this example the authors identify some of the key design challenges in developing these systems. This is followed by a discussion on future research directions and necessary developments in the design and implementation of Long-term and Post-Acute Care systems.
