Diffusion-weighted EPI using Siemens Vision was applied in 276 cases with cerebral vascular disease. Acute cerebral infarction up to 22 days after the onset showed high signal on DW-EPI. DW-EPI with 20 second acquisition time is superior to CT scan and T2WI in cases of acute cerebral infarction. Intracranial hematoma showed a homogeneous high signal within one day after the onset. Marginal low signal of the hematoma afterwards. Cytotoxic edema around the hematoma was recognized in two cases out of 25.
