Abstract
Background
he study was done at a community hospital and it's belonging counties.
Material:
The trial consists of 15 patiens, 12 family members of the patients and 20 healthcare workers from the hospital staff and the primary healthcare service. The questions were closed with optional answers and 2 questions open. There was also a more thorough interview of 4 patients.
Results:
It was emphasised from the patients and the family members that there was insufficient information from the hospital. The patients felt that they were not participating in the planning of their life situation after the stroke. Lack of communcation between the hospital staff and the primary healthcare workers. The family members of the patient felt overlooked. The interdisciplinary cooperation was working poorly across of the distribution levels.
