The CHSP is administered by CDC's NCCDPHP School Health Branch, formerly known as the Division of Adolescent and School Health (DASH). In 2012, NCCDPHP reorganized and DASH was transferred to the National Center for HIV/AIDS, Hepatitis, STD, and Tuberculosis Prevention (NCHHSTP). Those programs within DASH that were specific to nutrition, physical activity, obesity, and asthma remained in NCCDPHP within the new School Health Branch of the new Division of Population Health.

In the 2012 NCCDPHP reorganization, the Division of Adult and Community Health (DACH) was split into two divisions: the Division of Community Health (DCH) and the Division of Population Health (DPH). The DCH continues to operate and manage NCCDPHP community health programs such as the Community Transformation Grants (CTG), Racial and Ethnic Approaches to Community Health (REACH), and the Healthy Communities Program. DCH also ran the communities initiative of the Communities Putting Prevention to Work program.
