In Praise of Headache: A Farewell to psalms
It has been my custom to write once a year and thank those who make the Journal function so well. It has been a pleasure for me to do so and I do it no less enjoyably this year, although I would confess tinged with some regret for this is my last such message. Cephalalgia has published much in recent years that sings praise to headache, and often cited, could be seen as a collection of psalms (lowercase in deference to David's who influence me) to the subject.
Let me start by thanking our referees who are the backbone of the scientific effort. Tirelessly and without more than the intellectual reward of the task, year on year, and 2008 no exception, they have served the field by their thoughtful, helpful and constructive comments. The field is richer for their efforts and I thank them, as do the Associate Editors and indeed I think many authors who often remark how useful reviews can be. On this thought, the Journal is remarkably democratic and so it is that I thank the Associate Editors. Each brings particular expertise, considerable judgement and a dedication that gives us good turnaround, sensible decisions and a very broad coverage of our field. Lastly for the Journal let me thank you the reader, and the many authors who submit and publish their work here. We see some of the very best submissions in our field; it is a pleasure to read and facilitate their publication. I realize, as I have remarked in the past, that no one submits work for rejection. I can assure you that everyone, myself and the Associate Editors included, have had manuscripts rejected from the Journal. I would not pretend that I was jollier being rejected from this than any other journal, but it is the process; flawed but usually fair. Over the last years when it has not worked I am sorry and I take responsibility.
Since this is my last editorial I have the option, unfettered, to look back and muse, and then look forward. When I was first enticed to headache by Jim Lance we had ergotamine, we used methysergide and phenelzine, and diydroergotamine had just started to be employed in hospital. I grew up neurologically in a world dominated by a giant who taught me headache was both biologically fascinating and clinically an opportunity to do great good. It was a shock to me when I went abroad to find not everyone thought that way, a shock I have never really gotten used to. I can confirm it is biologically fascinating—these pages pay testament to that; and I hope we have all done a little good clinically so far. How wonderful an opportunity we have to build on what has been, to dissect the mechanisms at the bench, to translate these to brain imaging and clinical trials, and then to apply this knowledge in clinic. As we end the first decade of the twenty-first century we have at least two new totally novel approaches to treatment, calcitonin gene-related peptide (CGRP) receptor antagonists and 5-HT1F receptor agonists; treatment trials in prevention ongoing, and neuromodulation approaches across a range of intractable primary headaches previously greeted with, at best, a serious shake of the head. I see such a bright future for those of us involved and more importantly for our patients.
Lastly, it has been, without doubt, one of the singular highlights of my career, a distinct honour and a great opportunity personally to edit this Journal. I can only thank most sincerely everyone involved since 2000 when I took over, and wish the new team ever greater good fortune in the future. Cephalalgia has been a great affair, with some scars of battle as befits the job, and much pleasure to compensate. One certainly leaves the Editor-in-Chief role walking in the rain, just a little … a Farewell to psalms, as you could say.
