This case study describes the transvaginal sonographic diagnosis of a unilateral tubal ectopic pregnancy in dichorionic twins. The separate but unruptured gestational sacs are located adjacent to each other in the right fallopian tube; one fetus shows fetal cardiac motion, whereas the second fetus shows demise. In this case, transvaginal sonography is essential in diagnosing an ectopic pregnancy. Transvaginal scans should be utilized in combination with transabdominal sonography to make accurate diagnoses.
