Brainstem is commonly involved in spontaneous intracerebral hemorrhage (sICH). We report a 54-year-old man with accelerated hypertension presenting with ventral pontine hemorrhage. He had right sixth- and seventh-nerve palsy with left-sided hemiparesis. During the hospital stay, he developed disabling tinnitus in the right ear. Tinnitus is associated with the disruption of the central auditory pathway. He was given pregabalin for tinnitus and reported a significant improvement.
