Continuous positive airway pressure (CPAP) is generally applied through an endotracheal tube to improve oxygen in respiratory failure. We present a case demonstrating the use of CPAP by mask to reduce the intrapulmonary shunt produced by nitroprusside therapy. Oxygenation can be improved in selected patients by the use of mask CPAP, eliminating the need for intubation and mechanical ventilation.
