Abstract
The managerial aspect of providing quality, cost-effective health services to hospital patients is becoming increasingly important. However, merely focusing on cost reduction and quality improvement may jeopardize personnel satisfaction. Therefore, a balanced approach based on multiple considerations is necessary. Operating rooms (ORs) account for almost 33% of a hospital’s budget, and nursing labor costs represent more than 40% of a hospital’s budget. The concerns around health-care quality, personnel motivation, staff satisfaction, patient length of stay, and patient flow rate are greatly influenced by these two units with regard to day-to-day operations management. A possible approach is to look into the related essential problems, such as the nursing staff time scheduling in nursing units, and the doctor time scheduling in ORs. After surgery in the OR, patients stay in the subsequent ward for a period of time. This causes loading interdependency between the anterior OR and the posterior ward nursing unit; as a result, an attempt to schedule these two departments simultaneously is necessary for operation efficiency enhancement, service quality improvement, hospital budget reduction, and personnel satisfaction. In order to achieve the multiple objectives describing the conflicting priorities and differing preferences within nursing units and ORs, a compromise programming is adopted in this study as a balancing tool for multi-objective optimization to realize improved OR and nursing unit scheduling.

Introduction
Health-care systems involve a great amount of material and human resources. Thus, managers must pay close attention to efficiency improvement and cost reduction in their health-care systems. 1 –5 Additionally, the delivery of health care while enhancing the levels of quality service, patient gratification, and staff fulfillment is increasingly becoming a vital concern of administrators in health-care systems. Because operating rooms (ORs) and nursing unit are closely allied departments, both are considered simultaneously by administrators in their search for quality improvement and cost reduction. To this end, this study proposes an integrated model using multi-objective optimization for simultaneous nursing unit and OR scheduling.
However, ORs contribute the largest amount of revenue to most hospitals, influence the length of a patient’s stay in a hospital, and have a direct effect on the patient flow rate and bed occupancy level of a hospital. Thus, hospital administrators are interested in finding effective ways of managing an OR with enhanced efficiency and quality. Managing an OR scheduling is challenging because of the differing priorities and preferences among staff, 6 and because of the lack of costly resources for available facilities and personnel in most health-care systems. These reasons clearly stress the need for efficiency and demand the development of adequate scheduling in ORs. Various performance criteria are used to evaluate OR planning and scheduling procedures. In this study, performance measures, such as patient waiting time, OR utilization, and stability of occupancy level, are considered. Mathematical programming approaches are frequently used to solve OR planning and scheduling optimization problems. One reason for their success is the ability of commercial software to rapidly find solutions. Based on the aforementioned statements, Ozkarahan 7 employed the goal programming model for scheduling, with idle time and overtime being minimized, and with the satisfaction of medical staff members and patients being maximized. Chaabane et al. 8 introduced OR scheduling with operation time on a positive integer basis. Ogulata and Erol 9 applied mathematical programming with numerous criteria to formulate weekly OR schedules. With additional consideration of patient mixture, Vissers et al. 10 provided a mixed-integer linear program for cardiothoracic OR scheduling. Augusto et al. 11 used the Lagrangian relaxation method to minimize the makespan of the scheduling problem. Testi et al. 12 developed an integrated approach to improve the effectiveness of OR scheduling, simultaneously considering overtime, flow rate, and waiting time for patient admission. Lamiri et al. 13 provided a stochastic model for OR scheduling with overtime, idle time and related costs being minimized under the conditions of elective and emergency demand. However, the OR scheduling in their work assumed that the measurement unit of operation time was hours. Jeang and Chiang 14 built a model for OR scheduling with overtime and idle time being minimized under the constraints of doctors’ off duty time and outpatient consulting hours.
An appropriate level of nursing skill and proper nursing hours can provide quality patient care to ensure that a patient is discharged from hospital as soon as possible. The reduction in patient length of stay further improves the patient rate of flow and increases the unit utilization rate in a hospital. Consequently, service quality improvement and running cost reduction are realized. Besides a hospital’s economic, utilization, and quality considerations, nurse scheduling in the modern work environment is more concerned about personnel job satisfaction. 15,16 Nurses can normally express general preferences and related requests. The preferences of a nurse schedule take into consideration the distribution of work days and rest days, and the length of the working period, and the period of days off. 17 An unsatisfactory schedule typically consists of sequences of too many consecutive working days or too few days off and entails a penalty cost. The optimization techniques used in nurse scheduling include heuristic models, 18 integer programming, 19–20,37 stochastic programming, 21 fuzzy set theory, 22 nonlinear integer programming, 23 goal programming, 24 the multi-objective optimization problem, 25,26 and so on. In this study, a multi-objective optimization using compromise programming is adopted to consider the performance measures of nursing scheduling, such as nursing utilization, preference score, and nursing monetary values.
Harten et al. 27 indicated the practices for OR scheduling in conjunction with other relevant units. Due to the interdependence between the OR and nursing work, if an independent OR is scheduled, it often leads to unsatisfactory decisions and to a lack of care. Martinelly et al. 28 examined the connection between surgical arrangements and nurse scheduling for OR management, noting that considering both simultaneously when scheduling offers better resource usage and lower costs. However, their work is concentrated in the same department, for example, the OR synchronization involves two resources, OR and caregiver scheduling management. Such problems can be formulated as a mixed-integer programming. The model considers both the surgical schedule and the nurse schedule. The presented model considers the surgery scheduling in an anterior OR, and the nursing scheduling in a posterior ward at the same time. Because patients are removed from the OR after their operations, and are cared for by nurses during the length of their stay in a subsequent ward, the resolution of the problems related to the connection between anterior doctor scheduling in ORs and service scheduling in posterior nurse units is the main concern of this study. Thus, there exists an interdependency where a postsurgical patient will stay in the subsequent ward for a length of time before being discharged from the hospital; this stay causes side effects such as patient demands and nursing hours of nursing units in that ward. Specifically, the patient demands on a particular day in the subsequent ward depend on the type of operation and the days surgery scheduled. If the focus is only on the nursing schedule in the subsequent ward, ignoring the earlier OR schedule, the favorable decisions made will only be local optimizations. The authors’ recent work 14 includes the scheduling problem in the form of mathematical programming with the objective being to minimize the deviation between the total operation time and the total available time only in ORs. However, his work is still concentrated in the same department. In these regards, this study aims to create a scheduling solution for both the anterior OR and posterior nursing unit simultaneously. The integrated model is formulated with multiple objective optimizations via compromise programming.
Model development
Instead of the sequential scheduling decisions made in conventional approaches, the proposed approach considers the anterior OR scheduling and the posterior ward nursing unit rostering simultaneously. After patients’ operations are complete based on preoperating room scheduling, the patients are transferred from the anterior OR to the subsequent ward and cared for by nurses during the length of their stay. Clearly, the transferred patients cause patient demands, which in turn affect the required nursing staff level of the subsequent ward. Consequently, the affected nursing staff level continuously influences the nursing unit rostering in a ward. The length of stay in the recovery room is not taken into account in the proposed approach.
The objective functions for operation room scheduling
Various performance criteria are used to evaluate OR planning and scheduling activities. In this study, they are distinguished as utilization, waiting time, and leveling stability. Below, they are discussed in detail.
Utilization should be optimized, since underutilized and overutilized ORs result in poor service quality and unnecessary operation costs. Many studies elaborate on this trade-off and evaluate procedures based on OR efficiency, a measure that incorporates both the underutilization and the overutilization of the OR. OR scheduling is very unstable, and operation cost exhibits very large variation. The objective formulation for utilization is expressed as:

The second objective concerning OR scheduling is to deal with the problems of waiting time before surgery. Because complaints from patients and doctors about long waiting times are among the most common complaints in a hospital, reducing the waiting time has been an issue in many current research efforts. 29 In this study, it is assumed that a doctor’s outpatient hours and off duty hours are known in advance. Patient waiting time, therefore, is the remaining factor of concern in the OR scheduling. The weights, Vj , represent the degree of importance to patient j in waiting for admission. The scale of weight should be assigned before calculating the penalty score due to patients or doctors waiting for admission. The objective formulation for waiting time is expressed as:
The third objective in OR scheduling is the stability of the occupancy level. This is clearly understandable because postsurgical patients will be transferred to the subsequent ward for their period of stay. Consequently, the type of operation and the day of surgery determine the occupancy level in a ward due to later effects. Jeroen and Erik

propose several models for building operation schedules, while studying their impact on bed occupancy and minimizing their influence on bed shortage. As a result, decisions about bed capacity and occupancy, the nursing staffing level and time scheduling of a ward are based on a preschedule carried out in an OR. Since the nursing staffing level is known, as is the number of patients waiting for surgery, the remaining concern is the variability of bed occupancy level, which usually causes demand fluctuations in the related units. Because of its indirect influence on the effective utilization of resources, this fluctuation has a very strong impact on the cost-effectiveness of staff utilization and facility arrangements within the health-care system. Thus, the stability of bed occupancy level is also one of the major concerns in health-care management. The objective functions for nursing staff scheduling
Nurse scheduling is a critical issue that many hospitals must deal with. The main objective of nurse scheduling is to prepare a work schedule that meets the manpower requirements of the wards while satisfying various legislative requirements. The manpower requirements are dependent on patient demands during the patient’s stay after surgery. Nurses usually express their individual preferences to their supervisor, who tries to satisfy their preferences during the nursing schedule. As a scheduling task consists of on-duty and off-duty assignments on a daily basis along the time horizon, taking into account the variety of conflicting interests or multiple objectives between hospitals and nurses is essential for nurse scheduling. For instance, possible preferences include desired days off, preference for doing a certain shift on a certain day with a certain nurse, and so on. There may also be a demand for a certain nursing skill levels to satisfy quality requirements in each shift. Conventionally, nurses work on three shifts, day, evening, and night shift, and have some days off duty. Much of the work on nurse scheduling is concerned with cyclical scheduling, in which each nurse works a cycle of n weeks, where n is the length of the scheduling period. In this study, the schedule covers a period of 2 weeks. For scheduling purposes, the day shift is represented by 1, evening shift by 2, and night shift by 3. Generally, the nurses do not work the same shift throughout the 14 days, but rather rotate shifts to ensure even distribution of work duties among all the nurses. Unless specifically requested, the nursing supervisor will try to ensure that everyone has a similar number of different shifts.
For multi-objective optimization in nursing scheduling problems, three objectives are considered in the proposed approach. The multiple objectives are the utilization of scheduled nursing levels in fulfilling patient demands, nurse preferences in satisfying their day off arrangements, and cost-effective nurse scheduling. They are discussed in the following paragraphs.
The first objective is to consider the utilization of nursing staff levels for scheduling arrangement. The time gap between nursing hours resulting from nursing staff levels and patient hours resulting from an assortment of patient demands should be minimized. Because the different skill levels of nurses cause differing nursing hours, nursing hours (clock hours) must be converted, based on their skill levels, into the equivalent patient demand hours for health-care service. The number of nurses and their different skill levels result in the total equivalent patient demand hours. Daily equivalent patient demand hours depend on the different patient demands for all patients staying in a specific ward on a particular day. Nurse staffing levels must respond to the daily patient demands. Because the patient demand differs at different intervals of the day, evening and night shifts, the daily patient demands are distributed into shifts proportionally, expressed as:

The second objective is to consider nurse preferences in satisfying their day off requests. Each nurse has a number of days that they preferred to have off. The degree of nurse preferrence for a desired day off is measured with scores given by the nurses themselves. Because, in the nature of preference scores, the higher the score, the stronger the preference, the preference scores should be as high as possible for the most desired day off. Each nurse will tag their desired days off with a set of scores. For fairness in day off assignment, the sum of alloted scores should be the same for all nurses in a ward. For example, it is allowable for any nurse to allot the highest score as nurses like to ensure that the most favorable day off can be scheduled as long as the total assigned scores are the same for all nurses. For the sake of consistent expression in multi-objective optimization, the maximum of the preference scale is converted to minimzation with a minus sign being added to the objective function, thus representing a preference score. The third objective is to consider cost-effective nurse scheduling with quality care service ensured. In general, there are three classifications of nurses in this study: senior nurses, junior nurses and assistant nurses. The main differences among them are payment and skills. A higher payment is allotted for the skilled nurse who can expertly address the needs of patients in care. Normally, nurse salaries vary and are dependent on classification. In addition, extra pay is added to normal salary wages for evening shift work or night shift work. Thus, when calculating a nurse’s daily payment, the additional subsidy of those shifts must be taken into account. Obviously, different combinations of nurse classifications will result in different payments. The single objective functions, Z 1, Z 2, Z 3, Z 4, Z 5, and Z 6, will be further used for multi-objective formulation by compromise programming in the following model development. However, the proposed approach is still applicable when objective functions are defined for different considerations, or when the number of objective functions is greater than six, as in this presented case.
The constraints for OR scheduling and nursing staff scheduling
Because OR scheduling has already been introduced in previous work, 14 the related constraints would be found without repetition. Thus, the major contents in this subsection are those of nursing staff scheduling.
Other than the concern that occupancy level stability be included in operation room scheduling, another constraint, that the total number of beds occupied not exceed the capacity allowed in a particular ward, should be included in the problem formulation. The following expression represents the occupancy related constraint:

A three shift system is implemented here where a single day is split into three parts; day, evening, and night shifts. equation (8) is used to avoid assigning a nurse to work multiple shifts in a day.
To ensure the even distribution of nurses’ shifts and to avoid nurse dissatisfaction with shift allocation, nurses must not work a night shift immediately followed by a day shift. In addition, the maximum length of consecutive days that a nurse can be on duty is also constrained. 31 These concerns are expressed in the next two equations:
The nursing staff level established must be capable of providing sufficient care to meet patient demands for each shift. Because different skill levels of nursing create various nursing hours, which reflect the ability to provide satisfactory patient care service, nursing hours must be converted, based on skill levels, into equivalent patient hours for care service. Obviously, the equivalent patient hours from the part of nurses’ clock hours must be greater than the patient demand hours for each shift, as given in equation (11):
An additional constraint is the allocation of the appropriate number of nurses in various shifts to ensure that the quality service is achieved.
Another concern in nurse scheduling is the different patient care offered by female and male nurses. For example, when installing catheters for female patients, the patient care is normally performed by female nurses. Thus, a minimum number of females must be staffed for a ward in some situations. The following constraint, equation (13), is implemented to ensure the minimum number of female nurses,
For each shift, staff members’ skills and qualifications need to be considered. For example, at least one highly experienced nurse must be assigned to the evening shift. Thus, the following constraints are provided to meet the minimum qualified staff level for the necessary cooperation, skill and quality performance during the health-care provision. equation (14) represents the minimum number of senior nurses,

Multiple objective formulation for simultaneous optimization
Multi-objective optimization problems are generally solved by combining the multiple objectives into one common scalar objective, whose solution is a Pareto optimal point for Multi-objective Optimization Problems. Most of these combinations are either in a linear form or in a distance derivative. 32,33 Some of the relevant methods for solving optimization problems are weighted aggregation, global criteria, minimum deviation, and compromise programming. The latter is adopted in this study.
Decision-makers are often confronted with the need to optimize different criteria simultaneously, like minimizing staff cost and maximizing personnel utilization, while minimizing waiting time and maximizing care quality, and so on. Mathematically, the problem with such multiple criteria or objectives may be expressed as:
Another well-liked approach to handling multiple criteria or objectives is to select one of the objectives as the primary objective and confine the other objectives as constraints, expressed as:
However, optimal solutions are indeed true under the chosen weights we or limits CLe . Obviously, the obtained optimal solutions are based on the selection of weights we or limits CLe . In addition, since the objectives may be of different dimensions, a normalized measure is needed to make the individual objectives mutually commensurable. It is therefore necessary to use relative rather than absolute deviations for multiple objective optimizations. The method of compromise programming is similar to other normalization-based techniques. 34,35 The identified solutions are closest to the ideal solution as determined by some measure of distance. In the multi-objective optimization problem, it is obvious that sub-objectives will conflict with each other. In general, it is difficult to find a solution that is the best solution for all sub-objectives. Multi-objective programming focuses on the trade-offs of target conflicts. It explores how multiple objectives can conflict with each other to enable decision-makers to find a compromise solution that is closer to the ideal solution from a set of non-inferior solutions. That is to say, the basic spirit of compromise programming is to compromise each sub-objective according to its own needs and find the compromise solution closest to the ideal solution. Thus, compromise programming is adopted as a tool for multi-objective optimization in this study. For compromise programming, the overall multi-objective minimization can be expressed as:

In general, q is 1 or 2 for most applications. In this study, q is 1, indicating that the multi-objective value is summed linearly from the individual objective value. The multi-objective optimization is formulated with equation (36) as the objective function, and equations (7) to (20) as the constraints. The formulation can be solved by GAMS software. 36 GAMS is an abbreviation of General Algebraic Modeling System. It is an advanced modeling system for mathematical optimization. For linear and nonlinear programming problems, GAMS uses the Modular In-core Non-linear Optimization System (MINOS) algorithm. The MINOS algorithm combines the reduced gradient method with the quasi-Newton method. GAMS makes it easy to create, modify, and debug input files for optimized models, and the input files are compiled into a format acceptable to lower-level optimized numerical algorithms. After the algorithm program is executed, an output file containing optimal values can be obtained.
Application
Operation room scheduling
A practical example, from the department of gynecology in a teaching hospital, is used to demonstrate the proposed model. The planned period covers 10 working days, weekends excluded, over a 2-week span: k is 1, 2,…, 10. Thus, nk is 10. There are 8 doctors in the department, 50 patients waiting for operations, and 2 ORs provided. Doctor i is 1, 2,…, 8, patient j is 1, 2…, 50 and OR r is 1, 2. So, nd , np, and nr are 8, 50, and 2, correspondingly. By referring to Table 4, of the 50 patients, numbers 2, 10, 21, 29, 39, and 44 are urgent patients, while numbers 3, 16, 25, and 43 are emergency patients. To measure the penalty score as a result of waiting for admission under the given OR scheduling, the scores 1, 3, and 5 are assigned to represent the degree of importance for admission. The ORs are open between 8:00 a.m. and 5:00 p.m. daily. Thus, the available operating time, TD , is 9 h for each OR. The morning duration TA is 5 h, and the afternoon duration TP is 4 h.

After their diagnoses during outpatient consulting hours, some patients will be scheduled to undergo a particular surgery performed only by their doctor within the planned period. Each patient is allocated to one of the two ORs as applicable. It is possible that two operations can be performed by the patients’ doctors in different ORs. Based on the data on operating times for each doctor over the past three years, the mean operating time for each operation may be obtained, and is summarized in Table 4.

Nursing scheduling
In this section, a practical example from the same department as that of the OR scheduling is used to demonstrate the proposed model. There are 18 nurses in the department; nurse f is 1, 2,…, 18. Patient care is required for the entire week despite operations being performed only on weekdays. In this example, the planned period is cyclic for 14 days: w is 1, 2,…, 14. Thus, W is 14. Nurses are scheduled for the entire week including weekends, as with days 6, 7, 13, and 14, which are normally not scheduled as operation days. The nurses’ rostering is three shifts for 24 h a day. The shifts s are 1, 2, and 3 representing day shift from 8.00 a.m. to 4.00 p.m., evening shift from 4.00 p.m. to 12.00 a.m., and night shift from 12.00 a.m. to 8.00 a.m. correspondingly. Assuming that a nurse working for 8 h each shift can provide 6 patient hours, the remaining 2 h are allocated for non-direct patient care affairs.
Eighteen nurses, 17 females and 1 male, in a nursing unit are classified into three categories depending on their skill levels. Clearly, Fe is 17 and Fm is 1. The first category for senior nurses has eight nurses (f is 1 to 8) with a daily wage of about NTD3500 (New Taiwan Dollar). The second category for junior nurses has five nurses (f is 9 to 13) with a daily wage of about NTD2500. The third category for assistant nurses has five nurses (f is 14 to 18) with a daily wage of about NTD1800. An extra payment Ps is added to the regular payment, Cf, when nurses work evening or night shifts. In this example, the total payment for an evening shift is 1.10 times the regular payment. Similarly, the total payment for night shifts is 1.35 times the regular payment.
The hospital administration stipulates that each nurse must have eight days off per month. The planned period covers 14 working days cycling over a 4-week span. Each nurse has 4 days off in 14 days. Because there is competition among nurses in the selection of preferred days off during the planned period, it is necessary to construct a distribution rule for fairness among the nurses. In this example, each nurse is allowed total preference scores (K) of one hundred points. The degree of competition in winning the particular days off depends on how much a nurse is willing to give. The total scores from all nurses in a unit must be optimized so that a satisfactory schedule is reached for the entire unit. The assigned scores for all nurses are summarized in Table 6. For fairness in distribution, the assigned number of evening shifts and night shifts for each nurse must fall within a restricted range. In this example, each nurse must be on duty for at least two evening shifts and one night shift during the planned period. The maximum number of evening and night shifts are six and four, respectively, for each nurse. To ensure adequate quality of patient care, the minimum numbers of nurses are six for the day shift, two for the evening shift and two for the night shift. The minimum number of female nurses,
Table 7 shows the patient status, 1 or 0, during the patient’s length of stay. For example, the patient has to stay in hospital for six days in a type 1 operation. Then, the status in six days is 1, 1, 1, 1, 1, and 1. The rest of the days, from 7 to 11 days, are 0. The values of

Table 8 summarizes the daily patient hours for each operation type during the patient’s stay. For example, in regard to a type 1 operation, as mentioned above, the daily patient hours during their stay are 0.50, 1.50, 2.00, 1.00, 0.50, and 0.25 h during the period of 6 days. The first day of the patient’s stay, m 1, takes 0.50 h for preparation before their operation the next day, m 2. Another example illustrates the following conditions: patient number j = 14; doctor i = 3, operation type t = 1, showing that the patient is admitted on Thursday (second week) and undergoes surgery on Friday (second week). Equations (37) to (41) form part of the methodology in the formulation of equations (3) and (4). The longest length of stay is 11 days regardless of operation type. For the sake of convenient communication and formulation, Tables 1, 2, 3, 7, and 8 are formed as eleven days as a basis, and this study sets 0.00 h as representing no patient hours. In fact, 0.00 h can also be interpreted as no more nursing care needed after the patient has been discharged. Similarly, Tables 7 and 8 are also tabulated using eleven days as a basis. The patient hours shown in Table 8 are replaced with 0 or 1, representing the patient status as “discharged” or “stay.” Of course, when patients stay in the hospital, beds are occupied until they are discharged. G max is 30 beds.
The day shift has the majority of the total number of patient hours; the evening shift has fewer patient hours, and the night shift has the minority of patient hours. The total patient hours are obtained by summing patient hours from the column in Table 3. Because the patient demands are mixed as shifts vary, the total patient hours must be further distributed according to shifts assigned. In this example, H1, the day shift, occupies 80%; H2, the evening shift, 15%; and H3, the night shift, 5% of the total hours in a day w. For example, the total hours of patient demands for day 1 are 47 h. According to the proportion defined in the aforementioned statement, 37.6 h are distributed for the day shift, 7.05 h for the evening shift, and 2.35 h for the night shift, respectively. Clearly, for quality care, the nursing hours for the allocated nursing staff level in each shift should be more than these patient hours. This is why equation (11) is added to the constraints.
Numerical results
The GAMS mathematical program software was used in the finding of the optimal solutions; it took about 193.9 s to run the program for the proposed problem. The flow chart of the simultaneous OR and nursing schedule is shown in Figure 1. The numerical results of single objective are shown in Table 9. Table 11 is the OR scheduling for rooms 1 and 2. For example, doctor 1 and patient 3 are arranged for an operation in room 2 on the morning of day 3. Table 12 is the timetable of doctor i and patient j arranged for an operation in room r. For example, in the first row, doctor 2 and urgent patient 10 are scheduled for OR 1 between 8.00 a.m. and 10:42 a.m. on day 1. Table 13 is the timetable for doctor 1 for all his patients during the planned period. Table 14 is timetable for all nurses in the planned period. The schedule for nurse 17, with a special request, is [1, 1.2, 0, 0, 0, 1, 1, 1, 3, 3, 0, 1, 2]. Table 15 indicates the corresponding nurses on duty on a specified shift on a particular day. Discussion
This study aims to create a scheduling solution for both the anterior OR and posterior nursing unit simultaneously. Because of the interdependence of OR and nursing unit, an integrated schedule for both units at the same time is essential for successfully running a hospital. Accurate and simultaneous master schedules are essential to ensuring that the surgical operations of a hospital proceed in an efficient manner, and that the nursing units provide appropriate care for transferred patients after their operations. Schedules influence the patients’ length of stay, the quality of patient care, patient flow rate, effective and efficient utilization, and HHR (human health resources) in a health-care system. A multi-objective function is formulated via compromise programming, which is similar to other normalization-based techniques, to find a compromise scheduling simultaneously for both units. The normalized measure is used to make the individual objectives mutually commensurable, thus more sub-objectives of different dimensions can more easily be considered at the same time. In addition, the formulation of a multi-objective function can be solved by GAMS software without complicated calculations. An application is used to demonstrate that the proposed approach is not only cost effective and satisfactory for hospital personnel, but also provides quality assurance and patient satisfaction.
Conclusion
Because of the interdependence of OR and nursing unit, an integrated schedule for both units at the same time is essential for successfully running a hospital. In additions, hospital managements are also confronted with the need to optimize different criteria simultaneously, like minimizing staff cost and maximizing personnel utilization, while minimizing waiting time and maximizing care quality, and so on. Based on these regards, the proposed approach is developed. The contributions are: (1) The performance criteria are distinguished as utilization, waiting time and leveling stability for anterior OR scheduling and utilization, nurse preferences, and cost-effective for posterior nursing unit scheduling. Because of dependence existing among the above performance criteria and between anterior OR and posterior nursing units, both units are planned at the same time. The performance criteria are not limited only on the above criteria. They can be expanded without damaging the developed model. (2) The model is formulated with multi-objective optimization problems which are solved by combining the multiple objectives into one common scalar objective through a compromise programming to find a compromise scheduling simultaneously for both units for unbiased decision making. (3) The proposed model can timely adjust time table under uncertain environment resulting from interruption such as doctors, nurses, and patients changing scheduling. (4) Accurate and simultaneous master schedules ensure that the surgical operations of a hospital proceed in an efficient manner, and that the nursing units provide appropriate care for transferred patients after their operations. Consequently, schedules reduce the patients’ length of stay, improve the quality of patient care, increase patient flow rate, enhance effective and efficient utilization in a health-care system.
