We report a case of extensor digitorum bowstringing after complete rupture of the extensor retinaculum treated successfully with an ankle extensor retinaculum graft. Reconstruction of structure, form and mechanical properties of the extensor retinaculum is possible owing to the histological similarity of anatomical pulleys throughout the body.
