A left carotid endarterectomy was performed under regional anesthesia in a patient who had recovered from a stroke that involved the right cerebral hemis phere. Arch angiography showed total occlusion of the right internal carotid artery with high-grade stenosis and a large ulceration of the left internal carotid artery. To minimize ischemic time, a modification of the standard procedure for insertion of the Pruitt-Inahara shunt was used successfully.
