Although pulmonary sequestrations are usually supplied by a large anomalous systemic artery, no previous report of aneurysmal change in this artery has been described. Our patient demonstrated a large aneurysm of the distal systemic artery which was filled with organized thrombus. Although both trauma and cystic medial necrosis could have been causative agents, both were ruled out and the etiology of this aneurysm formation remains speculative.
