This paper provides an overview of the concept, the functionalities and the architecture of the CHILDCARE platform. Our system aims at supporting the out-of-hospital continuous healthcare management of children and promoting its quality. All the involved actors in this domain are integrated across the system. Our focus is the utilization of the latest technology accomplishments towards the incorporation of teleconference sessions, collaborative spaces and intelligent features over a common secure platform. This combination allows: •remote communication between health experts and the child’s family• setup, execution and monitoring of health management plans• efficient collaboration between the responsible doctor and healthcare professionals• access from multiple devices from any place, at any time• role-based management of the involved actors with respect to their actual involvement in the healthcare management, ensuring confidentiality and security at all levels. The main components of CHILDCARE include • a mechanism for interfacing legacy systems• the virtual workspace for delivering communication functionalities• the intelligent features for accessing and utilizing information from the personal health records and the system repositories so as to implement added-value services• interfacing modules for ensuring appropriate interaction with respect to the role and interests of each user group.
