Abstract
An integrated home care delivery system supports the ongoing efforts of local health care providers to prepare for reimbursement changes by implementing critical processes and procedures in order to achieve increased value, cost-effectiveness, and process efficiency. With the centralization of intake and case management processes for a group of affiliated home care providers, the providers are presented with opportunities to reduce costs, increase customer satisfaction, and compete for managed care contracts. Success at such an initiative requires ongoing provider commitment, automation, and consistency of practice.
