Many systems come together to provide for the health care needs of an individual. Case management is practiced by several professional disciplines in various settings. It is in the best interest of the consumers to have case managers collaborate so that each professional specialty may be fully realized. The social work case manager in a managed care setting is a psychosocial resource in the health care continuum.
