Abstract
This article describes the key concepts involved in performance improvement, the cycle for improving organizational performance: design, measure, assess, improvement, and innovation. The performance improvement program for a home infusion pharmacy is described, acknowledging the need for the pharmacist to be an integrated health care team member who collaborates and communicates with patients, physicians, and nurses to achieve continuous pharmaceutical care. Two performance indicators that address the need for pharmacists to provide pharmacokinetics and nutritional clinical monitoring for home health care patients are described.
