Abstract
The use of various terms for critical paths (i.e., care plans, care mapping) creates problems for practitioners trying to understand the concept. It is important for pharmacists to understand their role in this process. Once pharmacists understand their role, they may take responsibility for their involvement in implementing and managing critical paths. Once responsibility is understood, the importance of documenting their role needs to be understood. Once the full process is understood, the pharmacist can participate in the development of critical paths and the necessary documentation. This article is intended to augment the process, as well as provide information on the outcomes of care provided.
