Urachal carcinoma represents less than 1% of all bladder neoplasms. A case is reported of a 66-year-old man who presented with episodes of macroscopic hematuria. Cystoscopy revealed the presence of a papillar neoplasm between the dome and the right bladder wall. The histological examination showed a “mucinous adenocarcinoma morphologically similar to intestinal neoplasms”. Heteroplasia of the urach was diagnosed following urography, CT and NMR.
