Malignant metastasis to the bones of the hand is rare. Metastases may be isolated and presentation deceptive. Routine bone scan involves only the axial skeleton and therefore does not rule out metastasis elsewhere. A specific request must be made to scan peripheral bones if this is the area of interest.
We present a case which illustrates these points and emphasizes the role of palliative surgery where other measures have failed.
