The evaluation and surgical treatment of 39 patients with hyperparathyroidism has led to the development of a systematic strategy with excellent postoperative results. Successful planning requires an awareness of the limitations of histopathology. This strategy incorporates a knowledge of embryology, anatomy, and endocrinology, as well as the technologic advances of autotransplantation and the cryopreservation of parathyroid tissue. Our surgical procedure for exposing the parathyroid glands and our philosophy of conservative removal of enlarged tissues only are discussed.
