In this article, there is an error in Note 1 on pages 82 and 83. The Note should read as follows:
1. In the simplest case of exposed groups observed for a given time span, let A1 be the number of deaths observed in an exposed group of size N1, for an average death risk (mortality proportion) of R1=A1/N1; let A0 be the number of cases observed in an unexposed comparison group of size N0, for an average death risk (mortality proportion) of R0=A0/N0. Then RR=R1/R0 is the risk ratio.
