Abstract
Standardization of informed consent documents can be problematic. Without guidance, investigators often submit unacceptable consent forms. This can delay both study implementation and institutional review board proceedings. One solution is to provide investigators with a basic informed consent structure on a computer disc. By using this template, the investigator can ensure that pertinent informed consent issues are addressed.
