This series of two videos reprises the opening session of the 2019 Annual Meeting of the American Thyroid Association. A summary of original research articles influencing surgical thyroidology, published from January 1, 2018, to September 30, 2019, is presented. The review is structured around the following six themes: (1) Who (should have surgery)? Patient selection for surgery vs medical management or observation. (2) What (operation)? Extent of surgery guided by preoperative evaluation (including molecular markers). (3) Where (should surgery be performed)? Surgical outcomes and quality. (4) When (to intervene)? The timing of surgery. (5) Why (should we operate or not)? New data regarding the risks and benefits of surgery. (6) How? Innovations in surgical technique and perioperative care.
