A 23-year-old male presented with persistent symptomatic primary hyperparathyroidism after two previous negative neck explorations. This case demonstrates the close embryologic relationship between the submandibular salivary gland, which originates from the endoderm lining the third pharyngeal arch, and the inferior parathyroid and thymus, which originate from the third pharyngeal pouch. Embryological symmetry had been maintained during embryogenesis in this patient, although with the right salivary tissue migrating with the thymus and parathyroid on the right, while the parathyroid migrated with the submandibular gland on the left.
