Lobular capillary hemangioma, formerly called pyogenic granuloma is a benign lesion that presents as a mass over the skin or mucosa and occurs most often as a result of trauma. The intravascular variant of this lobular capillary hemangioma, however, is rare and had been described earlier in the head and neck region by Cooper et al.

But such a variant arising within another hemangioma is extremely unusual. We present one such case in the pediatric age group, the first case of its kind being reported in the English-language literature. This highlights the probability that these lesions, although gathered under the broad title of the hemangiomatous group of neoplasms, their behavior is still uncertain.
