On page 734, in table 2, column 4 in the row ‘Work within support system’, should read +, not ++.
On page 735, in the section ‘Case management: does it work’, item 2 should read ‘is the intervention useful in applied clinical settings’, not used.
On page 737, column 2, paragraph 2, the fourth sentence should read ‘Evidence of effectiveness and cost effectiveness’, not cost-effective users.
On page 738, column 2, paragraph 3, the second sentence should end ‘(the Care Programme Approach [70,71]), not reference [63].
On page 739, column 1, paragraph 3, the last sentence should read ‘based on the ‘political motives’ of ‘vested interests’ [18], not reference [55].
On page 743, column 2, in the line 5 from the bottom of the page the words ‘and several’ should have been omitted.
On page 744, reference 17 should cite ‘Johnston S’ not ‘Johnston T’.
