Urinary stone disease is a common and often recurrent condition that can affect kidney function and requires a range of medical and surgical treatments, all of which can have a significant impact on patients' health-related quality of life (HRQoL) and treatment preferences.
To review the literature systematically for all studies that include HRQoL measurement or patient preferences in the context of urinary stone disease.
Ovid MEDLINE(R), Ovid MEDLINE(R) In-Process, EMBASE, SCOPUS, EconLit, and Web of Science were searched from inception to January 2016. All study designs with adult participants were included. Narrative synthesis was performed.
Thirty-five studies met the inclusion criteria (six randomized controlled trials and 29 observational studies) from 15 countries, including 5472 patients. Eleven studies showed that stone formers had worse HRQoL than the general population; it was noted that stone formers were more likely to suffer from depression. Women have significantly lower HRQoL than men. Twenty-six studies used a generic HRQoL measure and six were nonvalidated disease specific. Studies concerning patient preference were heterogeneous and showed that extracorporeal shock wave lithotripsy is still favored above other interventions and that a large number of patients would prefer the treatment pathway to be decided upon by their clinician.
Urinary calculi and its treatment can have significant negative patient impact and influence patient preferences. Patients with stone disease tend to have worse physical and mental HRQoL, quantified using generic measures. Structured research with disease-specific measures underpinned by sound methodology would be beneficial and aid in development of patient-centric management.

Ureteral stent-related pain is a well-known side effect of stent placement. To date, there is a paucity of resources that address this topic. Herein, we present theories on stent pain pathophysiology, summarize available pain outcome data for different stent designs, and provide an overview of the management of stent pain, including preplacement modifiers, medical management, and other considerations.
This narrative review focused primarily on articles indexed in the PubMed®, Google Scholar™, and EMBASE databases. No formal search strategy was used and no meta-analysis of data was performed.
Stent pain pathophysiology is multifactorial and likely a result of mucosal irritation along with retrograde reflux of urine. While there is a consensus on the lack of association between stent length, diameter, and stent-related flank pain, stents should be properly sized so as to prevent dislodgement. Insufficient data exist comparing stent materials and durometry. Multiple drug-eluting stents are in development and have demonstrated promising early results. Alpha-blockers have shown efficacious results and should be considered along with or in combination with anticholinergics and nonsteroidal anti-inflammatory drugs (NSAIDs) in the treatment of ureteral stent-related symptoms, with judicious consideration of their side effect profiles. Periureteral botulinum toxin A injections are a promising, novel treatment modality.
Ureteral stent pain is common and multiple modalities have been studied and are in clinical use for its treatment. Care should be taken to avoid placement of stents if possible, with continual reassessment of indications to maintain stents in patients. Relative heterogeneity among studies and small sample sizes make creating specific evidence-based pain management recommendations challenging. Alpha-blockers, antimuscarinics, and NSAIDs are all generally well tolerated and effectively reduce symptoms, but patient-specific factors must be the paramount consideration when choosing monotherapy or combination therapy. Future studies are needed to better define ideal material characteristics and pharmacologic treatments.
There are several positions in the operation of percutaneous nephrolithotomy (PCNL), such as prone position, supine position, flank position, and modified supine position for PCNL, but the supine and prone positions are the main two choices for several years. However, there is still discrepancy on the optimal position for PCNL. Therefore, we performed this meta-analysis to evaluate safety and efficacy of the supine versus the prone position in PCNL for renal calculi.
We searched MEDLINE, SCOPUS, and the Cochrane database libraries to look for relevant studies. All eligible controlled trials comparing supine versus prone positions for treating renal calculi were included in the meta-analysis. The main outcome of efficacy (stone-free rate, mean operative time, and hospitalization time) and safety (complication, blood transfusions) were assessed by using Review Manager 4.2 software. We calculated the estimate of effect associated with the two positions according to the heterogeneity using random-effects or fixed-effects models.
Thirteen studies (six randomized controlled trials and seven retrospective studies) with a total of 6881 patients contributed to this meta-analysis. The meta-analysis indicated/suggested that PCNL in the prone position was associated with a higher rate of stone clearance than PCNL in the supine position (odds ratio [OR]: 0.74; 95% confidence interval [CI]: 0.65, 0.84;
Compared with the prone position, the PCNL in the supine position has a slightly lower rate of stone clearance, albeit shorter mean operative time, and lower incidence of blood transfusions. The meta-analysis suggests that the PCNL in the supine position is a promising alternative.

The results of this study suggested that micro-perc and RIRS were highly effective methods for the treatment of moderately sized renal stones in children, with comparable success and complication rates. Patients and their parents should be informed about the currently available treatment options, and of their efficacy and safety. However, further clinical trials are needed to support these results.
Struvite stones have been associated with significant morbidity and mortality, yet there has not been a report on the medical management of struvite stones in almost 20 years. We report on the contemporary outcomes of the surgical and medical management of struvite stones in a contemporary series.
A retrospective review of patients who were treated with percutaneous nephrolithotomy (PCNL) for struvite stones at Duke University Medical Center between January 2005 and September 2012 identified a total of 75 patients. Of these, 43 patients had adequate follow-up and were included in this analysis. Stone activity, defined as either stone recurrence or stone-related events, and predictors of activity were evaluated after combined surgical and medical treatment.
The study included 43 patients with either pure (35%) or mixed (65%) struvite stones with a median age of 55±15 years (range 21–89 years). The stone-free rate after PCNL was 42%. Stone recurrence occurred in 23% of patients. Postoperatively, 30% of patients had a stone-related event, while 60% of residual stones remained stable with no growth after a median follow-up of 22 months (range 6–67 mos). Kidney function remained stable during follow-up. Independent predictors of stone activity included the presence of residual stones >0.4 cm2, preoperative large stone burden (>10 cm2), and the presence of medical comorbidities (
Struvite stones can be managed safely with PCNL followed by medical therapy. The majority of patients with residual fragments demonstrated no evidence of stone growth on medical therapy. With careful follow-up and medical management, kidney function can be maintained and stone morbidity can be minimized. Initial large stone burden, residual stones after surgery, and associated medical comorbidities may have deleterious effect on stone recurrence or residual stone-related events.
To compare three laparoscopic surgical techniques for the treatment of ureteropelvic junction obstruction (UPJO), assessing their difficulty, operating time, effectiveness, and complications.
The clinical histories of 54 patients with UPJO who underwent a laparoscopic procedure between June 2003 and September 2013 were reviewed. Anderson–Hynes (A-H) pyeloplasty was performed on 34 patients, nondismembered pyeloplasty on 11 cases (8 Y-V Foley plasty and 3 Fenger plasty), and cephalad vascular displacement or Hellström technique (HT) on 9 patients. The patients were selected for the different techniques depending on the findings during the procedures, according to renal pelvic size and the presence of crossing vessels. We compared the techniques according to intraoperative and postoperative outcomes. Complications were interpreted following the Clavien–Dindo classification. The success rate was defined as the absence of clinical symptoms and normal diuretic renography. Analysis of variance and chi-square tests were used for the statistical analysis.

Intraoperative observation of ureteropelvic junction allows selecting cases to undergo nondismembered pyeloplasty techniques, achieving similar results to A-H pyeloplasty, reducing operating time, complication rate, and hospital stay.

We retrospectively collected data of 155 consecutive patients who received RARC for muscle-invasive or high-risk nonmuscle-invasive urothelial carcinoma of the bladder between 2003 and 2014 at a high-volume robotic center. Diversion was performed intra- or extracorporeally according to the surgeon's preferences. Complications were graded according to the Clavien–Dindo system. Logistic regression analyses were used to assess the impact of age on postoperative outcomes.

Patients 80 years or older undergoing RARC for bladder cancer did not have a higher risk of peri- and postoperative morbidity and mortality rate and had a similar 3-year recurrence-free survival, suggesting that RARC can be safely performed in selected elderly patients by experienced surgeons.
To assess the feasibility and safety of a modified retroperitoneoscopic technique in supine position for the primary tumors located below the level of renal pedicle.
In this retrospective study, 27 patients with primary retroperitoneal tumors located below the level of renal pedicle were recruited from January 2009 till now. The mean age of patients was 49.0 years (36–66 years). The average body mass index was 21.1 ± 1.4 kg/m2. The retroperitoneoscopic resections were performed in supine position for all patients. The perioperative outcomes were collected and analyzed.
All cases were performed by the modified technique without converting to open surgery. The mean operation time was 98.3 ± 12.8 min, with a mean estimated blood loss of 94.4 ± 42.5 mL. There were four patients suffering from intraoperative complications, one with the inferior vena cava (IVC) injury, one with the lumbar vein injury, and other two with the peritoneum breach. Postoperatively, the median bowel recovery time was 2 days (range: 1–3 days), and the mean length of stay after operation was 2.9 ± 1.0 days. Our modified retroperitoneoscopic technique in supine position was proven to be safe and effective for the treatment of primary retroperitoneal tumors, especially suitable for the tumors located below the level of the renal pedicle.
Holmium laser enucleation of the prostate (HoLEP) is a well-established technique for the surgical management of benign prostatic hyperplasia (BPH). A significant number of patients who require surgery for BPH are being treated with anticoagulation (AC) or antiplatelet (AP) therapy. Other than slight prolongation of duration of bladder irrigation and hospital stay, the intermittent or continuous use of anticoagulant therapy did not adversely affect outcomes of HoLEP, suggesting that this approach is an attractive approach for such patients, especially when the prostate is extremely large.
Computed tomography (CT) is one of the most commonly used diagnostic modalities for urinary stone disease. In this study we developed a CT and clinical parameter-based prediction model for shockwave lithotripsy (SWL) outcome in proximal ureteral stones.
Data from 223 patients with single proximal ureteral stones treated with SWL between January 2009 and January 2015 were reviewed retrospectively. Clinical parameters including age, sex, body weight, and body mass index (BMI) were analyzed in combination with stone-related CT parameters (stone diameter, height, volume, location, Hounsfield units [HU], stone-to-skin distance [SSD]), and secondary signs (hydronephrosis, perinephric edema, and rim sign). Based on the cutoff values determined by c-statistics, a scoring system for the prediction of SWL outcome was developed.
The success rate was 65.9% (147/223), and in a univariate analysis body weight, BMI, SSD (vertical, horizontal), HU, stone diameter, height, volume, and all secondary signs were significantly associated with the success of SWL. BMI, stone diameter, and perinephric edema were independent predictors of SWL outcome and a prediction model based on these parameters will facilitate decision-making for SWL in proximal ureteral stones.
The role of α-blockers after shockwave lithotripsy (SWL) is controversial. The aim of our study was to evaluate the effect of tamsulosin and silodosin after SWL for kidney stones.
From 2012 onward, a consecutive series of patients undergoing SWL were prospectively enrolled and randomized by closed envelopes in three groups receiving tamsulosin 0.4 mg (A), silodosin 8 mg (B), and placebo (C) daily for 21 days after SWL. Anthropometrics, stone size, and location were recorded before SWL. Visual analogue scale (VAS) score was collected at 6, 12, and 24 hours after treatment to evaluate patients’ discomfort. Stone-free rate was assessed 1 and 3 weeks postoperatively. Complications and medical treatment–related adverse events (AEs) were recorded. Differences in VAS score, stone-free rate, and complications were compared among the groups.
Overall, 60 patients were enrolled. Mean stone sizes were 10.28 ± 2.46 mm, 10.45 ± 1.73 mm, and 9.23 ± 2.04 mm in groups A, B, and C, respectively (
Tamsulosin and silodosin are ineffective in increasing stone-free rate as well as early patients’ discomfort after extracorporeal lithotripsy.
Clinically significant urinary tract infection may be the most familiar complication of urinary catheterization; however, catheter-associated trauma can entail even more significant morbidity. We have designed and patented a novel atraumatic urinary catheter (AUC) and through this study, we compare its efficacy with the conventional Foley catheter (FC)
Forty male rabbits were divided into two equal groups for FC and AUC. The animals were catheterized on day 4 for 8 hours to compare the maintenance of the tubes and sedated before standardized forcible extraction of the catheter under sedation with the balloon still inflated. They were then examined visually and by cystoscopy.
In a number of animals in either group, the catheters failed to drain urine effectively due to anatomical issues. Those were excluded from the study. At the extraction phase, evidence of urethral trauma was found in 13 of 15 FC rabbits, but only in 4 of 14 in the AUC group (
The two catheters bear similar efficacy for maintaining urine drainage; however, traumatic injury is significantly less common following forceful extraction of the AUC than a conventional Foley.
