Mothers of preterm infants face significant challenges to breastfeeding. The theory of planned behavior (TPB) is a well-known framework comprising three domains (attitudes, perceived control, and social norms), which has been used to conceptualize the array of factors that influence health-related behaviors and develop interventions to promote behaviors.
We used the TPB framework to determine the array of factors that contribute to breastfeeding among mothers of preterm infants.
Using qualitative research methods, we conducted in-depth, semistructured interviews with mothers regarding their experiences feeding their preterm infants according to TPB domains. We developed themes based on an iterative process of review of transcripts and conducted interviews until thematic saturation was reached.
We interviewed 23 mothers in 3 states 2 to 6 months after hospital discharge; 22 mothers initiated milk production and 6 were breastfeeding at the time of the interview. Factors that were positive and negative toward breastfeeding were present for all three TPB domains. Regarding attitudes, mothers felt that breastfeeding was a way to bond, that breast milk was healthy and protective, and that breast milk alone was insufficient for a growing preterm infant. Regarding perceived control, mothers felt empowered to breastfeed due to encouragement from hospital staff, friends, and family, and had difficulty overcoming their infant's immature oral feeding skills, competing responsibilities, and perceived infant preference for bottle feeding. Regarding social norms, mothers reported support for and against breastfeeding among hospital and Special Supplemental Nutrition Program for Woman, Infants, and Children (WIC) providers, family, friends, and the media.
Interventional studies geared toward breastfeeding promotion among mothers of preterm infants may focus on addressing barriers to direct breastfeeding during the neonatal intensive care unit and early post-discharge time periods.
The rates of “any” human milk use in neonatal intensive care units have improved during the last decade. However, efforts to help mothers in expressing and maintaining mother's own milk (MOM) through discharge remain a concern. This study aims to improve MOM in preterm neonates.
The study was a quality improvement (QI) initiative conducted from May 2018 to April 2019. All preterm neonates <34 weeks were included. In the “initiation phase” (May 2018 to August 2018), a breastfeeding support group was formed and mothers were given structured antenatal and postnatal counseling regarding MOM using videos and leaflets. This improved MOM rates when compared with retrospective controls. In the continuation phase (August-2018 to April-2019), various plan-do-study-act cycles were conducted to test strategies such as frequent telephonic reminders, standardization of Kangaroo mother care, nonnutritive sucking protocols, and involving family members during daily counseling sessions. The initiation and continuation phases were compared with the retrospective baseline data.
Of a total of 125 preterm neonates, 27 were excluded for various reasons. Within 3 months of initiating the study, the proportion of neonates who received MOM within first 24 hours improved from 24% in retrospective controls to 82.9% in the initiation phase (
QI has shown promising results in improving MOM in preterm neonates.
The aim of this study was to evaluate the association between possible covariates, with emphasis on maternal body mass index (BMI), on exclusive breastfeeding and on total breastfeeding during the first postnatal year.
A longitudinal study encompassing 723 women who were followed during the first postnatal year
Data concerning pregnancy, delivery, neonatal period, and breastfeeding were extracted from respective medical records. Sociodemographic data on the participants were self-reported. The Cox Proportional Hazard Model was used for investigating the effects of different covariates.
Compared with women with BMI <25.0, obese women ran a higher risk of ceasing exclusive breastfeeding prematurely (Hazard ratio [HR] = 1.38,
Median duration of exclusive breastfeeding was shorter among obese women compared with women with BMI <30.0 (3.0 months versus 6.0 months). Corresponding figures for total breastfeeding were 4.0 months versus 8.0 months. Concerning parity and exclusive breastfeeding, there was no difference in median duration between primiparous women and multiparous women (4.0 months), whereas multiparous women had a longer median duration of total breastfeeding, than primiparous women (8.0 months versus 7.0 months).
The risk of ceasing exclusive breastfeeding prematurely is high among obese women. This result indicates the need for targeted supportive interventions, individualized according to BMI. With increasing age, the risk of ceasing breastfeeding prematurely decreases, and compared with primiparous women, multiparous women run a lower risk of ceasing exclusive breastfeeding prematurely.
An efficient method for measuring maternal milk production is needed. Our objectives were to: (1) validate a milk production rate (MPR) protocol in exclusively breastfeeding mothers; (2) determine MPR change following 48 hours of increased breast emptying; (3) assess agreement between MPR and infant test-weighing; and (4) characterize MPR in early postpartum exclusively breastfeeding mothers.

MPR is a valid measure of current maternal milk production capacity, but is not accurate for evaluating infant intake in exclusively breastfeeding dyads.
Breastfeeding is an important health concern for postpartum women.
This study aimed to investigate the effect of breastfeeding frequency on the level of serum prolactin (PRL), milk intake, and infant weight gain.
The time and duration of each breastfeeding episode were recorded by participants from day 1 to 28 postpartum. According to their diaries, we divided participants into the low-frequency breastfeeding group (Group I; <10 breastfeeding episodes/day) and high-frequency breastfeeding group (Group II; >/ = 10 breastfeeding episodes/day). A total of 23 mother–infant pairs were enrolled; blood samples were drawn between 1600 and 1800 hours. The PRL levels were examined using the DPC Immulite system.
Overall, 71.8% (23) of the enrolled mother–infant pairs completed the follow-up. Infant birth weight was higher in Group II than in Group I (3275.6 ± 93.3 g versus 2918 ± 82.1 g). On day 28 postpartum, infants in Group II ingested significantly more milk per feeding (71.6 ± 4.0 mL versus 54.1 ± 5.2 mL) and gained more weight from birth (142.9% ± 4.5% versus 130.2% ± 2.4%) compared with those in Group I. The mothers of Group II had significantly higher basal serum PRL levels (116.4 ± 11.8 ng/mL versus 72.7 ± 7.77 ng/mL), but a significantly lower increase in PRL postsuckling (168.5% ± 23.1% versus 291.6% ± 37.6% of basal PRL). Increase in frequency of breastfeeding of over 10/day is associated with baseline PRL levels and increased milk production and weight gain. These results provide useful information for breastfeeding women.

After adjusting for individual- and family-level sociodemographic and maternal-child health factors, living in a neighborhood with 4 amenities was associated with 1.54 (95% confidence interval [CI] 1.06–2.23) times the odds of BFI, compared to children living in neighborhoods with no amenities. There was a negative association (adjusted odds ratio [aOR] 0.83; 95% CI 0.70–0.99) between neighborhood social support and BFI, although living in a supportive neighborhood was associated with 1.37 (95% CI 1.11–1.69) times the odds of EBF6m. There was a negative association (aOR 0.71; 95% CI 0.54–0.93) between perceived neighborhood safety and EBF6m. The observed associations differed by race-ethnicity.
Community-level structural and social support factors influence breastfeeding outcomes, independent of previously described individual level sociodemographic factors, and the observed associations differ by race-ethnicity. These findings have implications for the development of “breastfeeding-friendly” communities and public policies.
Effects of breast milk (BM) enhancement on long-term BM feeding in preterm infants discharged from the neonatal intensive care unit (NICU) have not been examined previously. This study compares any BM feeding rates at 4 months corrected age (CA) in preterm infants discharged from the NICU on BM enhancement by fortification of expressed BM with infant formula additives (BM-F) versus BM supplemented with infant formula feeds (BM-S).
We compared infants born between January 1, 2013, and December 31, 2017, with gestational age <32 weeks or birth weight <1500 g, discharged home on BM-F or BM-S from two NICUs and followed at 4 months CA. Multivariate logistic regression with propensity scoring estimated the adjusted associations between nutrition at discharge and any BM feeding at 4 months CA.

In this observational study, preterm infants receiving BM fortified with formula powder additives at discharge from the NICU had better BM feeding outcomes without poor growth at 4 months CA relative to those infants receiving BM supplemented with infant formula feeds.
To examine the influence of exclusive breastfeeding on infant development among 3-month-old infants in a Chinese population.
Mothers and their 3-month-old infants were recruited from four maternal and child health hospitals from April 2018 to March 2019. Based on the infants' feeding patterns, the mother-infant dyads were divided into two groups: exclusive breastfeeding and formula feeding groups. Infant development was evaluated using the Chinese version of the Ages and Stages Questionnaires, 3rd edition (ASQ-C), and maternal depression, parenting confidence, and infant temperament were also assessed using the relevant scales/questionnaires. Multivariate logistic regression analysis was conducted to explore the effects of feeding patterns on infant development at 3 months.
The data from 417 mother-infant dyads were analyzed. For the breastfeeding group and formula feeding group, the risk of developmental delay measured by the ASQ-C was 4.1% (10/244) and 9.3% (16/173) respectively for the communication domain; 5.7% (14/244) and 8.1% (14/173) for problem-solving domain and 6.2% (15/244) and 12.1% (21/173) for personal-social domain, respectively. Compared with exclusive breastfeeding, formula feeding was a risk factor for delayed development of communication (adjusted odd ratio [aOR] = 2.60, 95% confidence interval [CI] = 1.42–4.75) problem-solving (aOR = 1.91, 95% CI = 1.06–3.45), and personal-social skills (aOR = 1.96, 95% CI = 1.12–3.42).
Exclusive breastfeeding is important for infant communication, problem solving, and social interaction at the age of 3 months. Formula-fed infants may be at a higher risk of developmental delay than exclusively breast-fed infants. It is necessary to encourage mothers to establish and continuous breastfeeding.
According to the Intergovernmental Panel on Climate Change, Greenhouse Gas emissions must decline by around 45% by 2030 and reach net zero in 2050. Biofuels, solar, and wind energy are obvious choices for reduction of the 75% of emissions from the energy sector (including transportation), but making reductions in the remaining 25%, the food sector, is more of a challenge. One way is to change our diets to increase low-carbon food alternatives.
We chose to examine the impact of powdered baby formula products. The aim of this study is to compute a minimal estimate of green house gas (GHG) emissions for powdered baby formula products sold in North America comprising Canada, Mexico, and the United States.
We found that in 2016, the North America Greenhouse Gas emissions (in tons of CO2 eq.) attributable to sales of powdered formula for Canada was 70,256, for Mexico, 435,820, and for the United States, 655,956. The North American per capita emissions based on infants and toddlers from birth to 36 months of age in 2016 was, at a minimum, 59.06 kg of CO2 eq.
The environmental and Greenhouse Gas impact of powdered baby formula, and related hazards arising from climate change, can be a relevant factor for health care providers in their advice to families on infant feeding. This study makes an innovative and potentially useful addition to the emerging evidence on this issue and should be considered when developing and funding infant and young child feeding policies and supportive programs.
