The current status of lithium in the treatment of mania and recurrent affective disorder is considered. The potential usefulness of lithium in clinical practice is not being realised. There is a need for information on clinical monitoring practices in the community. Long-term effects of lithium on renal function are reviewed and guidelines for monitoring are presented.
