Planning for medical care delivery demands epidemiologic expertise. Epidemiology needs conceptual expansion in order to meet this new challenge.
This paper sketches some aspects of the historical development of epidemiology. Three basic dimensions of morbidity are discussed: the biologic gradient of disease, duration (time), and the newer dimension of costs attached to various phases of the health delivery system.
