A case is reported in which there were multiple supernumerary premolars. The most remarkable feature of this case was the presence of bilateral premolar-form teeth between the lower first and second molars, a previously unreported site.
The treatment plan involved surgical removal of five extra teeth from the premolar or molar region in all four quadrants.
