Abstract
Members of school-level transition teams in four school districts in a predominantly rural state completed a mail survey (response rate of 58%, 24 of 41) regarding their experiences on the team. Respondents evaluated the effectiveness of school-level teams, identified barriers to transition success in the schools, and indicated the impact of written policies and procedures on team and individual activities. Results support the need for improved post-school case management and follow-up services, additional financial and transportation resources, and incorporation of career education models in the curriculum.
