Background: Retrobulbar hematoma is a rare and possibly devastating complication of orbital fracture repair, particularly when it occurs after the immediate postoperative period. Timely identification and intervention are essential to prevent permanent visual deficits. Case Presentation: We present an otherwise healthy 17-year-old female who experienced a retrobulbar hematoma nearly a month after left orbital floor fracture repair. With early identification of the problem and emergent evacuation of the hematoma, patient did not sustain any negative sequelae from the complication and recovered to her premorbid state. Conclusion: Delayed post-operative retrobulbar hematoma is a rare occurrence after orbital floor repair with few cases noted in the literature. Early detection and emergent evacuation are vital for avoidance of permanent disability. This case underscores the need for heightened awareness and prompt action for such complications.
