Abstract
This review article delves into the multifaceted realm of threesomes, examining their impact on societal norms and providing insight into the evolving landscape of human sexuality. It investigates various aspects including prevalence, motivations, configurations, and impacts, while emphasizing the significance of cultural acceptance and implications for relational well-being. Notable gender differences in participation and fantasies are explored, with an emphasis on how diverse configurations challenge established norms. Motivations for engaging in threesomes span from curiosity to intimacy enhancement, reflecting evolving attitudes toward sexual behaviors. The potential influence of threesomes on monogamous relationships is discussed, highlighting the importance of clear communication and safe sex practices. Additionally, the role of female agency and pornography in shaping perceptions and experiences is examined. Recommendations for researchers and mental health professionals underscore the need to address societal stigmas, explore cultural contexts, promote comprehensive sex education, empower female agency, and enhance communication skills. Understanding threesomes contributes to a deeper understanding of human sexuality, fostering inclusivity toward diverse sexual experiences and relationships. Further research is advocated to bridge existing gaps and cultivate a more empathetic society, recognizing threesomes as a complex aspect of human intimacy deserving of nuanced discussions and inclusive research.
Background
Threesomes, characterized by the involvement of three consenting participants with or without a romantic connection, have garnered increasing popularity worldwide. 1 This phenomenon encompasses a variety of configurations, from casual sexual encounters to long-term domestic partnerships such as polyamory, where three individuals form committed romantic relationships. Understanding threesomes involves examining their prevalence, cultural acceptance, motivations, and implications on individual and relational well-being.
In Western contexts, research indicates that approximately 18% of men and 10% of women have participated in a threesome at some point, with a significant percentage reporting fantasies about such experiences.1,2 Reportedly, 57% of Canadian men fantasize about mixed sex threesomes (MSTs) with two men and 85% with two women. 3 Such preferences emphasize the need to understand the content and evolving trends in MST. 4 However, societal stigma and methodological limitations often skew these estimates, underscoring the need for more robust research methodologies to capture a comprehensive picture. The exploration of threesomes in India necessitates a dual perspective, considering both historical and contemporary contexts.
Ancient Hindu literature references polyamorous relationships, suggesting that non-monogamous relationships are not solely modern phenomena. 5 Despite limited empirical data on the prevalence of threesomes in India, anecdotal evidence suggests they may be more common in urban areas, highlighting the importance of further research in this region. 5
Cultural shifts, including increased acceptance of unconventional sexual experiences and advancements in technology, have facilitated the growing prevalence of threesomes. Motivations for engaging in threesomes vary widely, from curiosity and sexual exploration to enhancing intimacy and addressing relational issues. 2 Understanding these motives is crucial for comprehending the evolving dynamics of sexual behaviors and relationship structures. Threesomes also present diverse configurations, challenging societal norms and stereotypes surrounding human sexuality. This diversity includes different gender compositions, frequencies of occurrence, and the nature of interrelationships among participants
Research on the cultural acceptance of diverse sexual orientations and its impact on attitudes toward threesomes contributes to the redefinition of traditional relationship norms and masculinity. 6 Examining the effects of threesomes on monogamous relationships, female agency, stigma, communication, and safe-sex practices sheds light on broader aspects of human sexuality and intimacy. By fostering inclusive and empathetic discussions, society can promote a deeper understanding of human sexuality, moving toward more respectful attitudes toward diverse sexual experiences. 1
Despite significant advances in research, there remains a substantial gap in understanding women’s perspectives on male-male-female (MMF) threesomes, often more stigmatized than other configurations. Recent studies highlight that women may find MMF threesomes enjoyable, especially when men engage sexually with each other, which can reduce objectification and foster a sense of security.7,8 Furthermore, the evolving sexual scripts in contemporary pornography and their influence on sexual behaviors and fantasies necessitate critical analysis to understand their impact on societal norms. 9
This article synthesizes recent research on threesomes, emphasizing the need for further exploration and inclusive discussions to promote a nuanced understanding of human sexuality. Recommendations for researchers and mental health professionals are provided, with a focus on addressing societal stigmas, exploring cultural contexts, and fostering comprehensive sex education programs to support individuals in navigating complex sexual relationships.
Main Body: Analysis and Discussion
In the main body, authors explore and discuss various aspects of threesomes, including their prevalence, why people engage in them, the different ways they happen, and their effects on individuals and relationships. Through our analysis and discussion, we aim to shed light on the diverse facets of this topic and what they reveal about human sexuality.
Research on threesomes reveals notable gender differences in participation and fantasies, with men more likely to engage in such experiences than women. A significant proportion of both genders report fantasies about threesomes, indicating a widespread interest in these encounters.2,7,8 In India, historical references to polyamorous relationships in ancient Hindu literature challenge the notion that non-monogamous relationships are solely modern phenomena.5,10 A recent cross-sectional conducted on university students in India found that 62% of respondents reported engaging with online pornography. The study also revealed various patterns of sexual behavior among respondents, with 31.3% reporting having no sexual partner, 28.4% having only one partner, 29.4% having 2-5 sexual partners, and 5.2% having more than 10 sexual partners. 11 Singh et al. (2020) analyzed the secondary data from Indian Demographic Health Survey (DHS) conducted in 2015-2016, which examined 1,69,814 unmarried women and 4,003 unmarried males, revealed that 18.4% of all relationships began before marriage, with 14% of unmarried women and 31% of unmarried men reporting premarital sexual relations with multiple partners. 5 Notably, 35% of unmarried youths aged 15 to 19 reported having several premarital sex partners. 5
These findings underscore the liberal attitudes toward sex among young people in India and their increased exposure to non-traditional cultural norms. The study suggests that such attitudes may contribute to negative sexual behaviors, particularly among adolescents. To address these challenges, comprehensive sexual education programs should be integrated into school curriculums to enhance understanding and awareness of sex and sexuality.
However, contemporary data on the prevalence of threesomes in India is limited, with anecdotal evidence suggesting a higher occurrence in urban areas. 11 This highlights the need for culturally sensitive research to understand the dynamics of threesomes in the Indian context. Recent research conducted at Panjab University, Chandigarh, provides insights into the prevalence of threesomes among university students, with estimates reaching up to 4.8% in a sample of 211 participants.5,11 This study underscores the relevance of exploring threesomes within the younger demographic and emphasizes the need for more comprehensive research in India. However, anecdotal evidence from platforms like Quora suggests that the actual prevalence of threesomes could be higher, especially in urban areas, than official statistics indicate.
Motives for participating in threesomes are diverse and multifaceted. They range from curiosity and sexual exploration to enhancing intimacy and addressing relational issues.1,2,12 For some individuals, threesomes provide an opportunity to explore sexual fantasies and desires that may not be fulfilled within a monogamous relationship. For others, they serve to strengthen emotional bonds and improve relationship satisfaction. Understanding these motivations is crucial for comprehending the evolving dynamics of sexual behaviors and relationship structures. 2
Threesomes exhibit diverse configurations, including different gender compositions, frequencies of occurrence, and the nature of interrelationships among participants. Common configurations include male-female-male (MFM) and female-female-male (FFM) threesomes. Recent research indicates that women may find MFM threesomes particularly enjoyable when the men are willing to engage sexually with each other, which can reduce feelings of objectification and foster a sense of security. 7 This challenges traditional stereotypes and highlights the complexities of human sexuality.
Engaging in threesomes can have varying effects on monogamous relationships. For some couples, it can enhance intimacy and communication, while for others, it may lead to feelings of jealousy and insecurity.6,13,14 Establishing clear boundaries and maintaining open communication is essential for navigating the potential challenges associated with threesomes. Research suggests that couples who engage in consensual non-monogamy, including threesomes, often emphasize the importance of trust and mutual consent to ensure positive experiences.8,12 Authors raise questions about the positive experiences resulting from morally unacceptable sexual interactions. Such interactions may lead to feelings of insecurity in a partner and fear of emotional intimacy. These relationships and encounters can have a negative impact on the sexual self-image and self-concept of both men and women. The authors recommend further research in this area to gather real-world data, especially at a time when there is a vast amount of pornography available on the internet.
Female agency and empowerment in the context of threesomes are critical areas of study. Women’s participation in threesomes can challenge traditional gender norms and stereotypes, promoting a more inclusive understanding of female sexuality. 15 However, societal stigma surrounding women’s sexual agency remains a significant barrier. Addressing this stigma requires fostering inclusive and empathetic discussions about diverse sexual experiences and promoting a deeper understanding of human sexuality (Scoats, 2019).
Effective communication and safe-sex practices are paramount in ensuring positive and healthy threesome experiences. Establishing clear expectations and boundaries can help mitigate negative emotions and enhance relationship satisfaction.7,8 Additionally, promoting safe-sex practices, including condom use and discussions about contraception, is essential for reducing the risks of sexually transmitted infections (STIs) and unintended pregnancies. Comprehensive sex education programs can play a crucial role in raising awareness about safe-sex practices and fostering responsible sexual behavior. 11
Internet pornography, with its vast array of content, plays a significant role in shaping sexual behaviors and fantasies. Threesome pornography has garnered substantial popularity, influencing individuals’ interest and participation in threesomes. 9 The content depicted in threesome pornography often reflects traditional sexual scripts, emphasizing aggressive behaviors and other-sex interactions. Understanding the impact of pornography on sexual attitudes and behaviors is crucial for comprehending the evolving dynamics of sexual norms in contemporary society. 16

Promote Female Agency: Encourage research highlighting women’s perspectives and experiences in threesomes, challenging traditional gender norms and stereotypes. Enhance Communication Skills: Emphasize the importance of communication and consent in navigating complex sexual relationships while providing resources and support for individuals and couples.
