Cystic enlargement of the optic nerve sheath is extremely rare and has been described in the literature in association with optic nerve meningioma.
We describe MR findings over a 3.5 year follow-up in a patient with a large slow-growing retrobulbar ONC, not associated with optic nerve sheath meningioma or other optic nerve or orbital disease.
