Embolizzazione preoperatoria dei meningiomi intracranici

Sono state messe a confronto due serie di meningiomi intracranici (78 pazienti in totale) omogenei per sede, dimensioni, istotipo, sesso ed età dei pazienti.

Una serie è stata sottoposta ad embolizzazione preoperatoria, dal febbraio 1992 al maggio 1996, con successiva eradicazione chirurgica da parte della stessa équipe.

La serie di confronto è stata individuata attraverso una ricerca retrospettiva che, per ottenere omogeneità di sede, dimensioni ed istotipo, si è spinta dal febbraio 1992 al marzo 1987. Le due serie di meningiomi, quella sottoposta ad embolizzazione e quella non embolizzata, sono state confrontate in rapporto al dato quantitativo delle sacche di sangue infuse durante l'intervento. Questi dati sono stati sottoposti ad elaborazione statistica. Gli autori presentano i risultati, discutono la metodica di embolizzazione, la morbilità e l'efficacia della stessa.

Gli autori discutono inoltre la valutazione dell'efficienza della metodica, intesa come capacità di far giungere gli emboli all'interno del parenchima tumorale ottenendo una devascolarizzazione ed una necrosi intratumorale. Sono discusse le tecniche di embolizzazione e le complicanze.

Gli autori concludono sottolineando la necessità di un ulteriore studio, possibilmente multicentrico, per valutare statisticamente il rapporto costo-beneficio dell'embolizzazione preoperatoria dei meningiomi intracranici. La valutazione della necrosi e della diminuita perfusione è stata affidata allo studio con RM pre o post embolizzazione (SE 400/20 prima e dopo contrasto paramagnetico e.v.), in tutti i 39 meningiomi. In casi selettivi si è ricorsi allo studio istologico del pezzo operatorio.
