Abstract
Endoscopy is increasingly combined with microsurgery in the treatment of cerebral aneurysms. Since December 1997, we have operated on 49 aneurysms, at our Neurosurgery Division, associating endoscopy with microsurgery in 38 cases: four in the posterior and 34 in the anterior circulation.
In our experience, endoscopic procedures improved visualisation of the anatomic relations between the aneurysm and nearby vascular and nerve structures; disclosed perforating arteries originating near to or behind the aneurysm or not displayed at angiography; identified possible compression of nerve structures by the clip; ascertained complete exclusion of the aneurysm sac and patency of the surrounding vessels after clipping.
