We report a case in which a fistula developed between the biliary system and the bronchial tree as a complication of a pyogenic liver abscess. The presenting radiologic sign of the fistula was blurring of the diaphragm and increased lung markings. There were no apparent clinical signs prior to surgery, and it is assumed that the fistula existed in a latent form. Formation of a fistulous connection between the biliary system and the bronchial tree due to pyogenic liver abscess is a rare condition of which only very few cases have been reported in the world literature.
