Two of the references and some of the values for the AVF2107g and PACCE studies have been updated in Table 1. An amended version of Table 1 is presented below.

Study acronyms are used for brevity: readers may refer to the cited references for full details of study names and details.

Also, the reference Hecht et al. [2008] on page 78 should be changed to Hecht et al. [2009] and the reference to Rosen et al. [2008] on page 77 to Hurwitz et al. [2009]. The Rosen citation should be removed from the bibliography and replaced with the new reference [Hurwitz et al. 2009] as presented in the references.
