Abstract
GPs work at the heart of their communities, endeavouring to deliver holistic care for their patients. The multidisciplinary and community-based way in which GPs work means that they are well placed to support patients to voice preferences for their future and end of life care. These discussions are often emotive and can be challenging, which may be why the uptake of advanced care planning (ACP) remains low. This article examines the legal framework of ACP, addresses practical considerations, and provides guidance on effective communication around the topic.
