Abstract
Shared care prescribing enables a hospital specialist to transfer certain aspects of a patient’s care, including the prescribing and monitoring of medication, to their GP. This process requires a shared care agreement involving the patient, GP and specialist, ensuring the patient is stable on the medication before the GP takes over. The arrangement includes ongoing specialist reviews, access to specialist advice for the GP, a clear plan of care with defined protocols and a statement of monitoring arrangements.
