The presence of dilated pupils that have no direct or consensual response to light after traumatic brain injury is an ominous sign, potentially heralding severe intracranial damage. We describe a case in which the presence of tinted contact lenses directly caused misinterpretation of the pupillary light reflex and discuss the clinical importance of this observation.
