Abstract
A number of industrial accidents of large magnitude and involving a large number of deaths or great financial loss have contributed to increased awareness that high technology poses great risks of accidents becoming disasters. Concurrently with, or perhaps because of, this increased awareness of the risks of high technology has come increased awareness that the organizations and people who manage this high technology often contribute to, rather than ameliorate, those risks. The symposium papers report on some research efforts to examine, understand, and evaluate the contributions of organizational and management to risk and safety.
