Abstract
Scenario analyses were carried out on 4923 industrial accidents in a refinery and chemical complex. Scenarios consisted of prior activity, accident event, and injury event. Five scenarios accounted for 16.4 percent of the accidents. One prior activity, assembling/disassembling equipment, was involved in three of the five scenarios. Two injury events, contact with a fluid (toxic or steam) and overexertion, were each involved in two of the scenarios. These findings along with other information about the accidents are useful for setting priorities and developing intervention strategies in safety programs.
