The authors analyzed worker behavior in the criticality accident which occurred on 30 September 1999 at a uranium processing plant in Tokai-mura, Japan. Five poor factors of organization were found to have influenced workers' behavior. In addition, the authors concluded that these factors are due to lack of organizational ethics regarding safety.
