A case of a large intracranial aneurysm is presented in which the dome distended dorsally to the internal carotid artery (ICA) but the neck was adjacent to the origin of the posterior communicating artery (PcomA). This case illustrates that aneurysms located on the dorsal ICA wall at the level of the PcomA include not only types occurring without relation to arterial divisions, as previously reported, but also rarer types growing from the PcomA junction.
