The authors report an unusual coincidence of jugular megabulb deformity and jugular vein ectasia with contralateral Meniere's disease. They stress the need for extensive radiologic evaluation in patients who present with pulsatile tinni tus or tinnitus that does not benefit from medical treatment of endolymphatic hydrops. Therapeutic measures are discussed.
