The authors present single photon emission computed tomography (SPECT) images in a six-year-old boy with right hemichorea and motor aphasia. Neither computed tomography (CT) scanning nor magnetic resonance (MR) images showed any abnormality. 133Xe SPECT showed only a mild low-flow lesion in the territory of the left middle cerebral artery(MCA). However, 123I N- isopropyl-p-[123I] iodoanphetamine (IMP) SPECT showed a definite low-uptake lesion in the left MCA territory including the striatum and Broca's area. The authors stress the usefulness of 123I IMP SPECT in detecting mild ischemic le sions and discuss the mechanism of occurrence of hemichorea.
