Use of the midline or parmedian incision in the extraperitoneal approach to structures in the pelvis and posterior abdominal wall is recommended for ease, better exposure and flexibility. Two cases are presented involving ar teriosclerotic occlusive disease of the external iliac artery, where a relatively minor procedure may render the patient asymptomatic at least for the im mediate and near future.
